Picture Day — Team photos (and individual photos if requested) will be taken on December 11, 2012 during practice. You can get an order envelope from your team mom. The envelopes should be returned on the day of the photos with payment included.


Game Day Attire — Coach Castle requests that on game days, the team comes to school in Game Day Attire. This means: Black long-sleeve dress shirt (for both home & away games), plus nice black pants, all-dark shoes, and tie. Red team ties with MV embroidery will be provided by Coach Castle. Otherwise, please wear a red tie for game day. No polo shirts or beat-up black sneakers, and no black sneakers with white soles, checkers, or any other decoration.


Water at Practice Our team athletic trainer, Chad Eichten, will make water available at practice each day. He requests that you bring your own reusable container.


Fundraising Opportunities — Please help support MVHS Boys Soccer! All fundraising proceeds will be deposited into the Boys Soccer ASB account. For information about this year's fundraising efforts, see the Fundraising page.


Forms Required to Play Soccer — There are 3 main forms required to play soccer. You will not be able to practice with the team until the folder is 100% complete including athletic physical and insurance information.

1. The Blue Folder (aka the Athletic Clearance Form). This folder will be mailed out to each accepted player/family. The folder itself is a form which includes:

2. The Athletic Physical Examination Form.

The annual Sports Screening Day at MVHS was held June 10, 2011. If you missed the screening, you will need to have your sports pyhsical completed by your family physician or at a clinic. You will need to print the Athletic Physical Form, have it completed by your physician, and turn it in to your coach before you can participate in any summer or regular season sports. Parent & Physician Signatures Required. 

3. The MVHS Emergency Information/Insurance Verification/Authorization to Treat Form. Please fill out completely and return along with the folder.

The form must be signed by both parent and student.

Please fill out all requested information, sign, date, and return the folder and its contents to the Parent Meeting on October 25, 2012, 7:00 pm.


In addition to the above forms, you are requested to pay SVUSD transportation fees for the season you are playing. For 2012-2013, the fee is $75 per sport. If you have not already paid this fee, please send a check to your son's coach made payable to "SVUSD" with your son's name and sport in the lower left corner of the check. For more information about transportation fees, please read the Co-Curricular Transportation Fees Letter (English & Español).

— You are encouraged to purchase an ASB card. If your son has already purchased one (at registration) there will be an "ASB" notation on his student ID Card. If he has not, he can purchase one at the ASB office. These fees help support our soccer program at MVHS.
